Job summary

Moments Hospice is seeking an RN Case Manager to coordinate high-quality end-of-life care across a patient caseload. The role includes hands-on nursing, patient education, and close collaboration with families and the care team.

Qualifications

• Licensed RN in the state you work in.
• Proficient with EMR software.
• Excellent planning, organizing, and communication skills.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
