Job summary

Moments Hospice is seeking an experienced RN Case Manager to deliver compassionate, patient-centered care to our home-based program. You will assess needs, coordinate plans, and work closely with families and the clinical team.

Qualifications

• Licensed RN in the practice state.
• Proficient with Electronic Medical Records (EMR) software.
• Strong planning, organization, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
