Job summary

Luminos Hospice is seeking an RN Case Manager to lead a patient care team delivering compassionate home-based hospice services. You will collaborate with the Clinical Director and a team of LPNs, LVNs, aides, MSW and a Chaplain to ensure optimal end-of-life care.

Ideal candidates hold a current RN license, with at least two years in clinical care and one year in hospice preferred. Strong assessment, documentation, and communication skills are essential as you support patients and families in our

Qualifications

• Nursing skills aligned with generally accepted standards of practice.
• Knowledge of state and federal regulations governing nursing and hospice care.
• Ability to work independently and make sound clinical decisions.
• Strong attention to detail and organized work habits.
• Excellent verbal and written communication with patients, families, and care teams.
• Good interpersonal skills to coordinate with clinicians, aides, social workers, and chaplains.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.
