Job summary

Adoration Health seeks a Registered Nurse to join our Home Health team in Chattanooga, TN. The RN will provide 1:1 patient-centered home care, follow physician orders, and support families in achieving health goals.

Responsibilities include assessments, care planning, medication administration, education, documentation, and on-call participation. A valid TN RN license, one year of nursing experience, and a driver’s license are required.

Qualifications

• RN license with current unrestricted registration in the state of practice
• One year nursing practice in a patient care setting required; home health/geriatrics preferred
• Valid driver’s license and proof of car insurance per Adoration policy
• New grads may be considered in select markets depending on program availability
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans aligned with MD orders and patient goals
• Direct nursing care: medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Education

Registered Nursing Degree (Associate or Bachelor)
