Serenity Hospice is growing and seeks an experienced Registered Nurse Case Manager to join our team serving patients and families in Chester County, PA. You will manage a caseload of hospice patients in homes, assisted living facilities, and skilled nursing facilities, delivering expert clinical care, symptom management, and education while coordinating the interdisciplinary team.

This role emphasizes compassionate, patient-focused care with a daytime schedule, no nights or weekends or on-call.

Qualifications

• Active Pennsylvania RN license in good standing.
• 1–2 years nursing experience; hospice/home health preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Reliable transportation and valid driver’s license.
• Ability to work independently while collaborating with a team.

Responsibilities

• Perform initial and ongoing assessments of patients and families.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing care, pain and symptom management, and medication administration.
• Educate patients and caregivers on disease process, self-care techniques, and end-of-life planning.
• Coordinate care with physicians, social workers, chaplains, aides, and volunteers.
• Document accurately and timely in electronic medical records.
• Participate in interdisciplinary team meetings.

Schedule: Full-Time – Monday through Friday –

No Nights No Weekends No On-Call Required

About the Role:

Serenity Hospice is growing and seeking an experienced and compassionate Registered Nurse Case Manager to join our dedicated team serving patients and families in Chester County, PA.

In this rewarding role, you will manage a caseload of hospice patients in their homes, assisted living facilities, and skilled nursing facilities, providing expert clinical care, symptom management, education, and emotional support while coordinating the interdisciplinary team to ensure the highest quality of end-of-life care.

Key Responsibilities:

• Perform initial and ongoing assessments of patients and families to determine hospice needs

• Develop, implement, and update individualized plans of care

• Provide skilled nursing care, pain and symptom management, and medication administration

• Educate patients and caregivers on disease process, self-care techniques, and end-of-life planning

• Coordinate care with physicians, social workers, chaplains, aides, and volunteers
