Job summary

LiveWell Homecare Agency, LLC is seeking a Hospice – Registered Nurse Case Manager to assess patient needs, develop and implement personalized care plans, and coordinate care across the healthcare team in home or SNF settings.

You will advocate for patients and families, ensure documentation of progress, and uphold evidence-based, culturally sensitive care. A nursing diploma or RN licensure is required, with a preference for Bachelor’s degree and 2–5 years of hospice experience.

Qualifications

• Nursing Diploma or Associate’s degree is required and RN licensure is essential.
• Driver’s license may be required for home-based visits.
• Less than 2 years of experience is preferred; Bachelor’s degree and 2–5 years experience are a plus.
• Emphasis on cultural competence, critical thinking, and safety awareness.

Role: Hospice – Registered Nurse Case Manager

Responsibilities: Assess patient needs, develop and implement personalized care plans, collaborate with the healthcare team, and communicate updates. Promote respectful, patient-centered care addressing emotional, spiritual, and cultural needs. Evaluate patient condition changes, update care plans accordingly, and ensure evidence-based practices for quality outcomes. Responsible for coordination and advocacy in a safety-sensitive role.

• Monitor patient progress and modify plans as needed.

Qualifications & Skills: Nursing Diploma or Associate’s degree, RN licensure, valid driver’s license, less than 2 years of experience (preferred Bachelor’s degree and 2-5 years experience). Emphasis on cultural competence, critical thinking, and safety awareness.
