Job summary

Hospice Services of Massachusetts is seeking an experienced RN Case Manager to plan, organize, and direct hospice care for patients and families. This on-the-road role requires a driver’s license and a reliable vehicle, with travel across Barnstable and Plymouth Counties. Up to $51 per hour, with weekly pay, reflects compensation for travel and responsibilities.

Qualifications

• Graduate of an accredited school of nursing.
• Active and unencumbered RN license in Massachusetts.
• 1–2 years of acute care experience in an institutional setting; 1–2 years in home care preferred.
• CPR certification.

Responsibilities

• Supervise hospice aides.
• Complete comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary team to ensure comprehensive care.

Full-time and part-time Hospice RN Case Managers
Company: Hospice Services of Massachusetts
We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.

Up to $51 an hour, depending on experience, with weekly pay.

This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel to portions of Barnstable and Plymouth Counties.
