Job summary

Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team. You will plan, organize, and direct the hospice care plan for assigned patients under the direction of the clinical director.

We offer a competitive hourly rate of $45 to $50, with weekly pay, and travel throughout Rhode Island is required. The role requires an active Rhode Island RN license and a nursing degree, with hospice experience preferred.

Qualifications

• Active RN license in Rhode Island
• Nursing degree from an accredited college or university
• Prior hospice experience preferred

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide a complete physical assessment and history of current and previous illnesses
• Supervise hospice aides
• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate patient status, write and initiate a plan of care, and re-evaluate needs
• Develop a care plan with goals based on nursing diagnosis and palliative actions
• Provide counseling to patients and families in meeting nursing and related needs

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide a complete physical assessment and history of current and previous illnesses
• Supervise hospice aides
• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate patient status, writing and initiating a plan of care, and regularly re-evaluating patient and family/caregiver needs
• Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
• Provide counseling to patients and families in meeting nursing and related needs

Requirements

• Active and unencumbered RN license in Rhode Island
• Nursing degree from an accredited college, university, or school
• Prior experience in hospice is preferred
