Job summary

Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team. As a Hospice RN Case Manager, you will plan, organize, and direct the hospice care plan for assigned patients under the clinical director.

We offer a competitive hourly rate of $45 to $50, with weekly pay, and travel throughout Rhode Island is required. Active Rhode Island RN license and nursing degree are required; hospice experience is preferred.

Qualifications

• Active RN license in Rhode Island is required.
• Nursing degree from an accredited college or university.
• Prior hospice RN experience is preferred.

Responsibilities

• Complete initial, comprehensive, and ongoing assessments of patients and families.
• Provide a complete physical assessment and history of illnesses.
• Supervise hospice aides.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status, plan of care, and re-evaluate needs.
• Develop care plans based on nursing diagnosis and palliative actions.
• Counsel patients and families to meet nursing needs.

Job Summary:

Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team! As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care plan for assigned patients under the direction of the clinical director. We offer a competitive hourly rate of $45 to $50, with weekly pay, and travel throughout Rhode Island is required.

Responsibilities:

• RN completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide a complete physical assessment and history of current and previous illnesses
• RN supervises hospice aides
• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate patient status, writing and initiating a plan of care, and regularly re-evaluating patient and family/caregiver needs
• Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
• RN provides counseling to patients and families in meeting nursing and related needs

Requirements:

• Active and unencumbered RN license in Rhode Island
• Nursing degree from an accredited college, university, or school
• Prior RN experience in hospice is preferred
