Job summary

Athena Health Care Systems is seeking a Hospice RN Case Manager for per diem work, coordinating care for patients in their homes and communities through our Athena Home Health & Hospice care team.

Responsibilities include supervising aides, conducting comprehensive assessments, and developing care plans with palliative nursing actions while counseling patients and families. Requires RN licensure in CT and CPR certification; prior hospice/home care preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in Connecticut.
• CPR certification.
• 1–2 years acute care experience; 1–2 years home care preferred.

Hospice RN Case Manager - per diem up to $49 an hour

As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

Responsibilities:

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process

• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations

• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions

• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate

• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements:

• Graduate of an accredited school of nursing

• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care

• Active and unencumbered RN license in Connecticut

• CPR certification

• Bachelor's degree, with one (1) year of hospice care experience preferred
