Job summary

Paradigm Health, a patient-focused hospice provider, seeks an experienced RN Case Manager. Full-time, Monday–Friday, 8:00 am–5:00 pm, serving 1–2 counties near the office.

You will perform comprehensive assessments, manage pain and psychosocial needs, coordinate with physicians and the care team, and educate patients and families. Indiana RN license required; Missouri openings available.

Qualifications

• RN license required; 2+ years RN experience.
• 1 year hospice experience preferred.
• Current driver’s license and reliable transportation.

Responsibilities

• Performs physical and psychosocial assessments on hospice patients.
• Assesses and reassesses pain; educates patients and families on pain management.
• Responds to physical, mental, social and emotional needs of terminally ill patients and families.
• Initiates plan of care, re-evaluates and updates as necessary and obtains physician orders.
• Initiates case conferences to coordinate multidisciplinary team responsibilities.

• Full-time Hospice RN Case Manager position
• Schedule: Monday thru Friday, 8:00 am -5:00 pm
• Available territory openings: Case Manager typically assigned to cover 1-2 counties near the Office.
• Performs physical and psychosocial assessment on all hospice patients.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
• Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
• Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
• Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc
