Job summary

Paradigm Health is seeking a full-time Hospice RN Case Manager to deliver compassionate, high-quality care to patients and families across a local Indiana territory. The role offers a Monday–Friday schedule, mileage reimbursement, and a strong benefits package within a culture recognized as a Top Workplace by Indy Star for six years.
