Job summary

Interim HealthCare of West Michigan is seeking a Registered Nurse for our Hospice division. This RN role offers flexibility and supports patient advocacy with a backing management team.

Qualifications

• RN with active, unrestricted license in the State of Michigan.
• At least 1 year of relevant RN experience (required).
• 1 year of home health experience preferred.
• Current CPR certification.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Assess patient physical and psycho-social status.
• Develop and revise the plan of care with IDG, patient, family, and physician.
• Educate and intervene to manage pain and symptoms.
• Ensure equipment, supplies, drugs and services are available.
• Update IDG plan as patient needs change.
• Document patient/family services accurately and timely.
• Attend IDG meetings regularly.
