Job summary

Interim HealthCare is seeking a Hospice Intake RN for a full-time, Monday–Friday office position. This role focuses on coordinating referrals, initial patient intake, and facilitating admission to care services.

You will interact with patients, families, and referral sources, ensuring timely initiation of hospice services. The role offers a stable in-office environment, weekly pay, and comprehensive benefits including health, dental, vision, and 401(k).

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure as a Registered Nurse (RN) in Michigan.
• One year of RN experience preferred.
• Hospice or home health care experience preferred.

Responsibilities

• Acts as a liaison with hospice referral sources and field staff.
• Receives and coordinates all incoming calls to provide basic information.
• Communicates with patients and families regarding initiation of care/services.
• Performs intake screening to support admission decisions.
• Receives and logs referrals and prepares reports.
