Salus Hospice is urgently seeking a compassionate Hospice Admissions RN to serve the Orange County region. This full-time role (with per diem available) covers Orange County and requires Vietnamese or Korean fluency.

You will participate in admissions, evaluate eligibility, obtain admission agreements, build referral relationships, and educate patients and families on hospice care. The ideal candidate holds a CA RN license, at least one year in hospice or home health, strong interpersonal

Hospice Admissions RN Responsibilities:

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process
• Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
• Obtain agreement for admission of referred patients and families who meet admission criteria.
• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Salus Hospice
• Maintain clinical and professional competence.
• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care.

Hospice Admissions RN Qualifications:

• Registered Nurse with valid CA state license
• Minimum one year of hospice or home health experience
• Strong background in medical/surgical, ICU/CCU, or ER is helpful
• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient’s place of residence
• Must have excellent interpersonal skills and a passion for hospice
• Understanding of hospice philosophy and needs of the terminally ill
• Self-directed with ability to work with little supervision
• Current Physical and TB test results
