Job summary

You will assess, plan, implement, and evaluate care, supervise hospice aide staff, coordinate services, and participate in interdisciplinary team meetings, delivering patient-centered care with empathy and integrity.

Qualifications

• Current unrestricted RN licensure in Ohio.
• Valid driver's license and auto insurance.
• Minimum of two years RN experience, preferably in hospice or palliative care.

Responsibilities

• Manage patient/family caseload, including assessments and care planning.
• Supervise and instruct hospice aide staff.
• Coordinate medication and supply delivery outside regular hours.
• Participate in interdisciplinary team meetings during on-call periods.
• Admit eligible patients for hospice care under physician orders.
• Report patient status and coordinate with healthcare teams.
• Ensure documentation and care notes are complete and accurate.
