Job summary

Elara Caring in Pennsylvania seeks a Registered Nurse Case Manager to advance care in home health and hospice. You will assess patients and families, coordinate interdisciplinary plans, and support caregivers across diverse settings.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to lift up to 50 pounds and travel between patient homes.
• Valid driver's license or reliable transportation for home visits.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual, and environmental domains.
• Monitor patient condition and promptly communicate changes to the physician and care team.
• Educate and support patients, families, caregivers, and staff on the Plan of Care.
• Provide education on hospice philosophy, pain, and symptom management to staff in various settings.
• Provide compassionate support at death and complete bereavement assessments.

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
