Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to lead compassionate in-home care across patient and family needs. You will assess psychosocial and environmental factors and coordinate with an interdisciplinary team to implement the care plan.

Strong communication with clinicians and family is essential to provide quality end-of-life care. The role requires RN licensure, 1 year of clinical experience, and ability to travel up to 50%.

Qualifications

• Graduate of an accredited School of Nursing (Certificate/Diploma/Associate/Baccalaureate).
• Current State License as a Registered Nurse (RN).
• 1 year of clinical care experience; hospice/home health preferred.

Responsibilities

• Assess hospice care patients and families to identify needs across physical, psychosocial, and environmental factors.
• Communicate significant findings to Clinical Supervisor and care team.
• Counsel and include patient and family in the Plan of Care.
• Teach hospice philosophy and pain/symptom management to staff in other care settings.
• Provide bereavement support at time of death.

As a Hospice Registered Nurse Case Manager, you’ll contribute to our success in the following ways:.

• Ensures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Provides appropriate support at time of death and perform bereavement assessment.

What is Required?

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
