Job summary

Elara Caring in Topeka, KS is seeking a Registered Nurse Case Manager to join our hospice and home health team. You'll assess patient and family needs across physical, psychological, social, spiritual, and environmental factors and coordinate care with physicians and the care team.

This role requires an active RN license, 1+ year of clinical nursing experience, and the ability to lift up to 50 pounds. Travel to patient homes is required, with benefits including comprehensive insurance, 401(k)

Qualifications

• Current state RN license required.
• Graduation from an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including patient transfers.
• Must have a valid driver's license or reliable transportation to travel between patient homes and work locations.
• Experience in hospice or home health is preferred.

• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
