Job summary

CareerNerds is seeking a Hospice RN Case Manager in the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak and Duncanville. This full-time role coordinates patient care and develops care plans with the interdisciplinary team.

You will perform head-to-toe assessments, monitor symptoms, and manage medications, while educating patients and families and ensuring timely documentation. Strong clinical judgment and communication are essential in this critical care setting.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized plans of care with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications
• Educate patients and families regarding condition, needs, and care
• Communicate patient progress with clinical leadership, physicians, and the care team
