Job summary

Joseph Michaels International is seeking a dedicated Hospice RN Case Manager to cover the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville. This full-time role focuses on coordinating and overseeing patient care within an interdisciplinary team.

The ideal candidate will hold an active RN license, CPR, and hospice experience, and possess strong assessment and communication skills to educate patients and families and ensure high-quality end-of-life care.

Qualifications

• Active RN license valid for practice in MO
• Valid driver's license
• CPR certification
• ≥2 years RN experience in clinical setting
• Hospice experience preferred (1 year FT)
• Reliable transportation and car liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, interpersonal skills

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized plans of care with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications
• Educate patients and families regarding condition, needs, and care
• Communicate patient progress with clinical leadership, physicians, and the care team
• Complete all required documentation accurately and on time

Hospice RN Case Manager

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville | Full-Time

Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience preferred
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills
