Job summary

Henderson Group Recruiting is seeking a dedicated Hospice RN Case Manager to join our client in the Dallas area south of Downtown Dallas, covering Cedar Hill, DeSoto, Red Oak, and Duncanville. This full-time position focuses on coordinating patient care and developing individualized care plans.

The role requires clinical assessment, symptom management, and collaboration with the interdisciplinary team to support patients and families.

Qualifications

• Current Registered Nurse license valid for practice in the state.
• Current driver's license and reliable transportation.
• Current CPR certification.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Strong written, verbal, organizational and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.

Hospice RN Case Manager

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville | Full-Time

Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience preferred
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills
