Job summary

Saint Cyr Partners is seeking a Hospice RN Case Manager to coordinate and oversee all aspects of patients' hospice care in the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville. This is a full-time role supporting an interdisciplinary team to develop individualized plans of care and manage symptoms and medications.

The RN will conduct assessments, educate families, and ensure timely documentation while providing compassionate, patient-centered care.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver’s license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.
