Job summary

JourneyCare Hospice is seeking a skilled RN Case Manager to join our growing care team in McHenry County, IL. This role oversees patient and family caseload, collaborating with interdisciplinary staff to deliver comprehensive hospice care.

The position requires an accredited RN program, active RN license, CPR/First Aid, and a valid driver’s license. Hours are Mon–Fri 8:30am–5:00pm with on-call every 12th weekend, in a supportive culture with extensive benefits.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient’s physical, psychological, social, and spiritual factors to assess needs and initiate interventions by the team.
• Initiate communication with the attending physician and interdisciplinary team for coordinating optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and reports; prepare care plans timely.
• Supervise and document delivery of care by Home Health Aide per guidelines.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
