Anew Hospice in Indiana is seeking a dedicated RN Case Manager to deliver compassionate hospice care in the Crawfordsville and Lafayette area. You will manage patient care using the nursing process and coordinate services within patients' homes and communities.

The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
