Anew Hospice is seeking an experienced Registered Nurse to provide comprehensive home-based hospice care in the Crawfordsville/Lafayette, IN area. The RN will assess, plan, implement, and evaluate patient care, coordinating with families and caregivers to ensure comfort and quality at home.

Ideal candidates hold an Indiana RN license with hospice or long-term care experience, and possess strong communication and community health education skills for in-home services.

Qualifications

• Valid Indiana RN license required.
• Hospice or long-term care experience preferred.
• Ability to work in patients' homes and communities.
• Excellent communication and assessment skills.

Responsibilities

• Deliver patient care using the nursing process in home settings.
• Plan, organize, and direct hospice services for individuals and families.
• Educate patients and families on health needs and care plans.
• Collaborate with community resources and care teams to coordinate services.
