Job summary

JourneyCare Hospice in Chicago, IL is seeking a Hospice Registered Nurse to provide comprehensive in-home and facility-based hospice care. The role includes patient assessments, care planning, and coordination with an interdisciplinary team to ensure high-quality end-of-life care.

Candidates must have an RN license, BSN preferred, CPR/First Aid, and ability to coordinate sensitive patient needs while maintaining compassionate family support.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Collect data from patient factors (physical, psychological, social, spiritual) to inform care planning and engage the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.

HOURS

Monday-Friday 8:30am-5pm and One Weekend Every 12th Week (4 weekends/year). 1 Winter Holiday and 1 Summer Holiday

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process


• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team


• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care


• Provide regular visits to re-assess the patient's needs and update the plan of care


• Perform clinical pronouncement of death and provide support to the family


• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely


• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
