Job summary

Care Initiatives Hospice seeks an experienced RN Case Manager to deliver compassionate, patient-centered care in home, long-term care, assisted living, or hospital settings across Iowa Falls, Eldora and surrounding areas.

Responsibilities include developing and overseeing plans of care, performing initial and ongoing assessments, coordinating with interdisciplinary teams, and providing education and support to patients and families. Iowa RN license and CPR required; CHPN encouraged.

Qualifications

• Active Iowa RN license is required.
• CPR certification required.
• Hospice or palliative care experience preferred.
• Two years of verifiable nursing experience (RN/LPN).
• Valid driver’s license and ability to travel across the region.
• Home internet access required.
• Ability to obtain Hospice and Palliative RN Certification (CHPN) within 24 months.

Responsibilities

• Deliver high-quality patient care in the patient’s home or chosen setting using clinical nursing skills.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies and procedures.
• Conduct continuous visits and interdisciplinary patient assessments.
• Provide counseling and education to patients, caregivers, and family as indicated.
• continually update the plan of care to meet patient-specific needs.

We're looking for a RN to cover the Iowa Falls, Eldora & surrounding areas.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
