Job summary

Care Initiatives Hospice in Sioux City, IA seeks an RN Case Manager to deliver compassionate, patient-centered care in home and community settings. You will assess, plan, and coordinate services with physicians and the care team to support patients and families at the end of life.

The role emphasizes comprehensive care across physical, emotional, and spiritual needs, traveling across Storm Lake, Pocahontas, Sac City and nearby areas.

Qualifications

• CPR certification is required for patient care.
• Hospice or palliative care experience preferred.
• RN/LPN with at least 2 years of verifiable nursing experience.
• Current driver's license and ability to travel within the region.
• Home internet access is required to support care coordination.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months.

Responsibilities

• Provide high quality nursing care to patients in their place of residence.
• Develop and oversee care plans under physician direction with interdisciplinary input.
• Perform initial and ongoing patient assessments per policy.
• Conduct continuous visits and interdisciplinary patient reviews.
• Document clinical activities clearly and accurately.

Care Initiatives is looking for a RN to help care for patients in Storm Lake, Pocahontas, Sac City & surrounding areas.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
