Job summary

Care Initiatives Hospice - Sioux City is seeking a compassionate RN Case Manager to provide in-home and facility-based hospice care across Sioux City area. You will work with patients and families to deliver individualized care focused on comfort, dignity, and quality of life.

Key duties include conducting initial and ongoing assessments, developing and updating the plan of care with the physician and interdisciplinary team, ensuring thorough documentation, and delivering education to patients

Qualifications

• Active CPR certification.
• Hospice or palliative care experience.
• Two years of verifiable nursing experience (RN/LPN) preferred.
• Valid driver's license and ability to travel between multiple locations within your region.
• Ability to obtain CHPN within 24 months of employment.

Responsibilities

• Deliver high quality patient care in the patient’s place of residence using clinical nursing knowledge and skills.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments following policies and procedures.
• Conduct continuous visits and interdisciplinary assessments of patients.
• Provide accurate clinical documentation and ensure clear communications.
• Offer patient, caregiver and family counseling and education as indicated in the plan of care.
• Maintain up-to-date plan of care addressing patient-specific needs.

Care Initiatives is looking for a RN to help care for patients in Storm Lake, Pocahontas, Sac City & surrounding areas.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
