Care Initiatives Hospice in Waterloo/Cedar Falls is seeking an RN Case Manager to deliver patient-centered hospice care. You will meet patients and families where they live—at home, in long-term care, assisted living, or hospital—and coordinate interdisciplinary services with the physician.

The role emphasizes compassionate, dignified care during life’s final stage, with opportunities for CHPN certification and growth through comprehensive benefits and a supportive team.

Qualifications

• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable RN/LPN nursing experience preferred.
• Current driver's license, automobile liability insurance, and ability to travel regionally.
• Home internet access.
• Ability to obtain Hospice and Palliative RN Certification (CHPN) within 24 months.

Responsibilities

• Deliver high quality patient care in the patient’s residence using nursing skills.
• Develop and oversee the plan of care with physician direction and interdisciplinary involvement.
• Perform initial and comprehensive assessments per policies.
• Provide continuous visits and interdisciplinary assessment.
• Deliver clear clinical documentation.
• Counsel and educate patient, caregiver, and family as indicated.
• Update plan of care to meet patient-specific needs.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
