Job summary

Care Initiatives Hospice is seeking an experienced Hospice Clinical Manager - RN to lead and support our Cedar Rapids clinical team. This role oversees patient care from referral through end-of-life, ensuring compliance, documentation, and coordination across care teams.

The successful candidate will hold an active RN license, have hospice or palliative care experience, and lead with compassion. CPR certification is required, and CHPN is preferred or obtainable within 24 months.

Qualifications

• Leadership or charge nurse experience in a healthcare setting.
• Hospice or palliative care experience.
• Active RN license in the operating state.
• CPR certification in good standing.
• Home internet access required; ability to work remotely if applicable.
• CHPN certification preferred or ability to obtain within 24 months.

Responsibilities

• Oversees visits from referral-to-admission and through death processes.
• Ensures quality care, documentation, and collaboration meet guidelines.
• Supports direct care coordination and hospice eligibility.
• Provides orientation, education, training and competencies as needed.

Key Responsibilities of a Hospice Clinical Manager:

• Oversees all visits from referral-to-admission and through death processes.
• Ensures quality care, documentation and collaboration are within guidelines.
• Supports direct care coordination, ensures hospice eligibility, and oversees recordkeeping functions.
• Provides orientation, education, training and/or competencies, as needed.
• Maintain and implement operational policies and procedures to meet patient and family needs in compliance with federal, state, and local requirements guidelines and Care Initiatives Hospice policies and procedures.
• Support and facilitate educational directives of the Administrator and/or corporate leadership.

What it takes to be a Clinical Manager with Care Initiatives Hospice:

• Active RN license (or ability to activate) in the state(s) in which you operate.
• Active CPR certification.
• Hospice or palliative care experience.
• At least one year in a leadership or charge nurse position or relative experience.
• Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.
• Home access to the internet.
• Hospice and Palliative Registered Nurse Certification (CHPN) preferred, or ability to obtain within 24 months of employment.
