Care Partners in Portland, OR, seeks a Case Manager, RN (Full-Time) to plan, coordinate, provide, and document hospice care from admission through discharge. You will work with physicians and the interdisciplinary team to tailor a hospice plan of care to each patient’s needs.
Responsibilities include skilled nursing assessments across settings, maintaining records, counseling families, and coordinating services. A valid OR RN license and ability to travel to patient homes are required.

This position is responsible for planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge. Utilizing physicians' orders and professional skills, and in coordination with the other members of the interdisciplinary team, the Case Manager develops, coordinates and implements a hospice plan of care that meets each patient's specific needs and is in compliance with Federal and State regulations, reimbursement guidelines, as well as the organization's policies and procedures
**New employees are required to complete tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
