Heartland Hospice seeks a dedicated Patient Care Manager to lead clinical excellence and supervise hospice services. You will oversee patient care delivery, manage visit scheduling, triage referrals, and chair Interdisciplinary Group meetings, reporting to the Executive Director or Administrator.

You will mentor staff, ensure compliance with Medicare/Medicaid/ACHC standards, and drive performance and quality improvements.

Competenze

• RN license in the state of employment is required.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.
• Demonstrated ability to apply hospice principles, regulatory standards (Medicare/Medicaid/JCAHO/ACHC).
• Knowledge and compassion for terminally ill patients and families.
• Ability to lead a diverse care team while adapting to emotional and clinical needs.
