Job summary

Ohio’s Hospice is seeking a Hospice RN Care Manager for a full-time daytime role based in Franklin, OH. The position includes M-F 8am-5pm and driving to patients in homes and facilities across Middletown and Butler/Warren counties.

The RN will plan and deliver care using the nursing process, collaborate with families and the health care team, and uphold professional standards in a compassionate hospice setting.

Qualifications

• RN's - Graduate of an accredited nursing school; ASN; BSN preferred.
• Active Registered Nurse license in the State of Ohio without any board actions.
• Two years of acute care nursing experience preferred.

Responsibilities

• Plan and deliver care to patients using the nursing process (assessment, planning, interventions, implementation, evaluation).
• Interact effectively with patients, families and the inter-disciplinary team.
• Maintain professional nursing standards and documentation while providing hospice care.

Hospice RN – Making every moment matter

• This is a full-time day shift position, M-F 8am-5pm.
• This is a driving position seeing patients that live in homes and facilities within Middletown and the Butler/Warren county area

As a Hospice RN you will plan and deliver care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interact with patients, families and other interdisciplinary team members while maintaining standards of professional nursing. We provide superior care and superior services to patients at their end-of-life journey. Only those who have a heart for hospice will succeed.

Hospice RN Qualifications:

RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years of acute care nursing experience preferred.

• CPR Certified
• Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
• Computer skills are sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
