Job summary

Ohio's Hospice is seeking an RN Care Manager in Columbus, Ohio for a full-time direct patient care role. The position requires nursing licensure in Ohio, CPR, and the ability to drive across Franklin County with weekend rotations and holiday requirements.

Responsibilities include planning and delivering care using the nursing process and collaborating with families and the inter-disciplinary team, while maintaining professional standards and compliance with background checks and drug screening.

Qualifications

• Graduate from an accredited nursing program
• CPR certification required
• Computer literacy to document care
• Valid driver's license; ability to drive in various conditions
• Must pass criminal background check and 10-panel drug screen
• CHPN certification preferred and available to Ohio's Hospice employees

What You Should Know About the RN Care Manager:

• This is a driving position serving Franklin County
• This is a full-time position, 40 hours/week: 8:00A-4:30P with weekend rotation every 6 weeks, and 2 holidays per year required.
• Collaboration with colleagues required.

The RN Care Manager's Essential Duties Are:

Plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.

RN Care Manager Qualifications:

• Graduate of an accredited nursing school
• RN nurse license in the state of Ohio without any board actions
• Two years acute care nursing experience preferred
• Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.
• CPR Certified
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime, or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
• Must be able to pass a criminal background check
• Must be able to pass a 10 panel drug screen
