Job summary

Brighton/Suncrest Hospice is seeking a hospice RN to work on-call with options for Full-Time and PRN schedules. The role focuses on admissions and patient visits in home, facility, or hospital settings. The RN will answer calls, manage symptoms, and advocate for quality hospice care as part of a dedicated care team.

The ideal candidate holds an active RN license, has at least one year of experience, and is comfortable with after-hours coverage and travel within the service area.

Qualifications

• Current RN licensure in the state(s) of practice with one year of experience.
• Admissions and On-call experience preferred.
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Time management to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.

Responsibilities

• Provide care and services for hospice patients at locations including homes, assisted living facilities, and hospitals.
• Complete hospice admissions and patient visits as needed.
• Answer calls from patients, families, and customers; respond to urgent needs and triage.
• Conduct nursing visits for symptom management, admissions, and end-of-life care.
