Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a full-time nursing role in Ardara, Pennsylvania. AHN Healthcare@Home emphasizes team-based care and collaboration to manage and deliver patient care plans.

We require an RN license in the state, an associate degree or higher, two years of RN experience (hospice preferred), a valid driver’s license, and the ability to travel about 20% of the day. A generous bonus package is included.

Qualifications

• RN license in the state you work.
• Associate degree or higher from an accredited School of Nursing.
• Two years of RN experience, hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• A commitment to consistently meet critical deadlines for charting.
• The skills needed to self-manage your time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.

• Provide management and delivery of patient care plans as part of a team-based hospice program.
• Collaborate with clinicians to ensure quality patient care and timely charting.
• Support flexible scheduling and adapt to patient needs.
