Job summary

Graham Healthcare Group is seeking a Hospice Registered Nurse (RN) to provide end-of-life care in patients' homes around Elmhurst, IL. This staff position offers a $10,000 first-year sign-on bonus and a standard 40-hour workweek with 8-hour shifts.

Responsibilities include patient assessments, symptom management, and family support, all while maintaining compliant EMR documentation and collaborating with an interdisciplinary team. A valid IL RN license and driver’s license are required.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Driver's license and ability to drive ~20% of your workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey with clear information.
• Ensure thorough, compliant EMR documentation and maintain safety standards.
• Travel to patient homes within the assigned territory.
