Job summary

VNA Health Group is seeking a compassionate Registered Nurse Hospice Liaison to assess hospice eligibility, coordinate transitions, and conduct admissions across Newark Beth Israel, Clara Maass, Cooperman Barnabas and Jersey City Medical Center. This pivotal role supports patient- and family-centered care.

You’ll collaborate with a multidisciplinary team to expand hospice access, facilitate smooth hospital-to-home transitions, and ensure clinical accuracy in admissions and documentation.

Qualifications

• Active New Jersey RN license; BSN preferred.
• Experience in hospice or community health nursing.
• Experience conducting hospice or homecare admissions preferred.
• Excellent communication, organizational and customer service skills.
• Proficient with MS Office; valid NJ driver’s license and vehicle.

Responsibilities

• Serve as bridge between providers and hospice patients for coordinated transitions.
• Conduct in-home and facility hospice admissions with clinical accuracy.
• Engage with community and clinical partners to increase hospice access.

Position Summary:

The VNA Health Group is seeking a compassionate and experienced Registered Nurse Hospice Liaison to join our dedicated Hospice Team. This vital role focuses on assessing patients for hospice eligibility, coordinating their transition to hospice care, and conducting patient admissions. The Registered Nurse Hospice Liaison will be responsible for covering the following hospitals: Newark Beth Israel Medical Center, Clara Maass Medical Center, Cooperman Barnabas Medical Center, and Jersey City Medical Center. As a frontline advocate for hospice services, you will foster continuity, collaboration, and trust among patients, families, and referring partners—all while supporting our mission of delivering patient- and family-centered care.

What You Will Do:

• Support the organization’s mission by striving for excellence and fostering positive interpersonal relationships
• Assess patient needs and provide counseling to patients and families during the hospice referral and admission process
• Conduct comprehensive hospice admissions, including clinical assessments, eligibility verification, and documentation
• Coordinate transition to hospice services to promote continuity of care
• Establish and maintain liaison with physicians, external partners, and internal care teams
• Monitor and support quality and appropriateness of services and programs
• Conduct community outreach to enhance hospice awareness and accessibility
• Assist in the development and implementation of a hospital discharge program at Community Medical Center
• Adhere to agency infection control and safety protocols and organizational policies

What You Will Need:

• Active New Jersey RN license (BSN preferred)
• Excellent clinical skills with experience in hospice and/or community health nursing
• Experience conducting hospice or homecare admissions strongly preferred
• Exceptional verbal and written communication abilities
• Excellent organizational, customer service, and interpersonal skills
• Strong computer proficiency, including MS Office Suite
• Valid NJ driver’s license, personal vehicle, and proof of insurance

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $51.00.
