Job summary

Visiting Nurse Association Health Group seeks a Registered Nurse – Hospice for full-time staff in Monmouth County. The role focuses on assessing patients, managing symptoms, and creating personalized end-of-life care plans with families.

You will work closely with clinicians and external providers to deliver compassionate hospice care, documenting status updates and adapting plans as goals evolve. A BSN and hospice/critical care certifications enhance candidacy.

Qualifications

• RN with Bachelor’s degree in Nursing.
• Hospice Palliative Care Nurse Certification and Critical Care certification preferred.
• 1+ year of supervised RN clinical experience.
• Excellent communication, organizational and interpersonal skills.
• Proficient computer and EMR systems skills.

As a Full Time Staff Nurse Hospice serving Monmouth County, you’ll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.

Responsibilities

• Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
• Assess patients with advanced illnesses and create individualized hospice care plans.
• Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
• Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
• Document patient status updates and adjust care plans as needs and goals evolve.

Qualifications

• Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred

Working Conditions/Physical Demand

Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license.
