Job summary

VNA Health Group in Monmouth County is seeking a Per Diem Staff Nurse Hospice to deliver compassionate, patient-centered end-of-life care. You will assess needs, manage symptoms, and create individualized care plans while supporting families with dignity and comfort.

The position requires an RN with a Bachelor's degree and 1+ year of clinical experience; certification in Hospice Palliative Care and Critical Care is preferred. Strong communication and computer skills are essential.

Qualifications

• RN with Bachelor’s degree in Nursing is required.
• 1+ year of supervised clinical experience as a registered nurse.
• Certification in Hospice Palliative Care Nurse and Critical Care skills preferred.
• Excellent communication skills, strong organizational and interpersonal abilities.
• Proficient computer and EMR skills.

Responsibilities

As a Per Diem Staff Nurse Hospice serving Monmouth County, you’ll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.

Responsibilities

How You’ll Move Hospice Care Forward:

• Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
• Assess patients with advanced illnesses and create individualized hospice care plans.
• Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
• Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
• Document patient status updates and adjust care plans as needs and goals evolve.

Qualifications

• RN with Bachelor’s degree in Nursing
• Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
• 1+ year of supervised clinical experience as a registered nurse

Working Conditions/Physical Demand

Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
