Job summary

VNA Health Group is seeking a Per Diem Staff Nurse Hospice to join our Monmouth County team. You will assess patients, manage symptoms, and develop individualized care plans while supporting families through the end-of-life journey.

Responsibilities include documenting updates, collaborating with clinicians, and providing education and emotional support to patients and families. A two-week orientation focuses on hospice practices and EMR use.

Qualifications

• RN with BSN required.
• Certification in Hospice Palliative Care Nurse and Critical Care preferred.
• 1+ year of supervised RN experience.
• Strong communication and organizational skills.

As a Per Diem Staff Nurse Hospice serving Monmouth County, you’ll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.

• Document patient status updates and adjust care plans as needs and goals evolve.

Qualifications

• RN with Bachelor’s degree in Nursing
• Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
• 1+ year of supervised clinical experience as a registered nurse
• Excellent communication skills
• Strong Computer skills
• Excellent organizational and interpersonal skills

Working Conditions/Physical Demand

• Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $51.00.
