Job summary

The Pennant Group, Inc. is seeking a Hospice Registered Nurse (RN) to support patients in their homes across the Las Vegas metro area. You will assess needs, craft care plans, administer medications, and provide compassionate support to families during life’s most tender moments.

Qualifications

• Current RN license in the State of Nevada.
• Minimum of 1 year nursing experience; hospice or home health preferred.
• Experience with hospice or home health software is a plus, not required.
• Strong clinical skills and patient-centered care mindset.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care and administer medications; manage symptoms.
• Educate patients and families on care techniques and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.

Requirements

• Current RN license in the State of Nevada
• Minimum of 1 year of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) and/or PointCare/PCC experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference
