Job summary

Comfort Home Health & Hospice in Las Vegas is seeking a compassionate Hospice RN to join our team as a Case Manager, delivering in-home care to patients across the Las Vegas Metro area.

You will guide families through care decisions, provide skilled nursing, manage symptoms, and maintain thorough clinical documentation, while working with an interdisciplinary team in a mission-driven, supportive environment.

Qualifications

• Active Nevada RN license required.
• At least 1 year of nursing experience; hospice or home health preferred.
• Experience with HCHB and/or PCC is a plus, not required.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and patient-centered care mindset.
• Ability to work independently and make a real difference.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans and revise as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares your commitment to excellence.

Requirements

• Current RN license in the State of Nevada
• Minimum of 1 year of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) and/or PointCare/PCC experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference
