Job summary

Affinity Hospice Management LLC is seeking an on-call RN to coordinate and deliver direct care for hospice patients in their homes and in partner facilities. You will support families, manage comfort and symptom control, and ensure proper documentation of care.

Qualifications

• Graduate of an accredited school of nursing with current RN licensure in Ohio.

Job Summary

The on-call RN is responsible for coordinating care, providing direct care/support for hospice patients and their families in the home and various nursing facility settings. The on-call nursing schedule runs weekends Every Fri 5pm-Monday 8AM. Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed.

Responsibilities and Duties

On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities.

Qualifications and Skills

• Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing

• Self starter, excellent time management skills, ability to multi-task in a fast paced environment, dependable, autonomous worker, current clinical skills, flexible, professional and tactful.

• CPR certification required.

• Holds valid driver's license and current drivers insurance coverage.
