Job summary

Affinity Health Management is seeking an on-call RN/LPN to coordinate care and provide direct support for hospice patients and families in home and facility settings. Flexible schedule, including night shifts, on-call duties, and weekend coverage.

The role requires graduation from an accredited nursing program, Ohio Board of Nursing licensure, CPR certification, and a valid driver’s license with active insurance. Travel to patient sites is expected.

Qualifications

• Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing.
• Self starter, excellent time management skills, ability to multi-task in a fast paced environment, dependable, autonomous worker, current clinical skills, flexible, professional and tactful.
• CPR certification required.
• Holds valid driver's license and current driver's insurance coverage.

Responsibilities

Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed.
