Job summary

Affinity Health Management is seeking an on-call RN to coordinate and deliver direct hospice care for patients in home and nursing facility settings. The role includes weekend on-call shifts from 10:00p to 10:00a, with a flexible schedule that can be full-time or part-time per diem.

The nurse does not carry phones; triage is handled by another nurse, and you go out as needed. Responsibilities include coordinating care with the hospice team, addressing comfort and symptom management, medication

Qualifications

• Must be a graduate of an accredited nursing program.
• Current Ohio nursing licensure is required.
• CPR certification required.
• Valid drivers license and car insurance are required.

Responsibilities

• Provide on-call support after agency hours for hospice patients and families, coordinating comfort and symptom management.
• Manage medications, DME needs, admissions, discharges, and ensure proper documentation.
• Coordinate care between the hospice interdisciplinary team and facilities.

Job Summary

The on-call nursing schedule runs every weekend from 10:00p - 10:00 a. Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed.

Responsibilities and Duties

On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities.

Qualifications and Skills

• Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing
• Self starter, excellent time management skills, ability to multi-task in a fast paced environment, dependable, autonomous worker, current clinical skills, flexible, professional and tactful.
• CPR certification required.
• Holds valid driver's license and current drivers insurance coverage.
