Job summary

Affinity Hospice Management LLC is seeking an on-call LPN to coordinate and provide direct care for hospice patients in home and facility settings. You will participate in after-hours triage by going out when needed, with a flexible schedule including night shifts and weekend on-call.

The ideal candidate is a graduate of an accredited nursing program, licensed in Ohio, CPR-certified, and capable of independent, compassionate care while documenting care and coordinating with the interdisciplinary

Qualifications

• Valid driver’s license and current auto insurance.

Responsibilities

Job Summary

The on-call LPN is responsible for coordinating care, providing direct care/support for hospice patients and their families in the home and various nursing facility settings. Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed.

Responsibilities and Duties

On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities.

Qualifications and Skills

• Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing
• Self starter, excellent time management skills, ability to multi-task in a fast paced environment, dependable, autonomous worker, current clinical skills, flexible, professional and tactful.
• CPR certification required.
• Holds valid driver's license and current drivers insurance coverage.
