Brighton/Suncrest Hospice is seeking a Registered Nurse to provide home-based patient care and support as part of an interdisciplinary team. The role may include Case Management, Admissions, or Resource duties across private homes, SNFs and assisted living facilities.

The position is weekend/part-time (Saturday and Sunday, 8am–5pm) with PTO, 401k employer match, and potential for position upgrades within a rapidly growing national company.

Qualifications

• Current RN license in good standing in the state(s) of practice.
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to travel across the service area with reliable transportation.

Responsabilités

• Provide care and services for a group of patients visiting their residences.
• Contribute as part of the interdisciplinary team with expertise in psychosocial assessments.
• Work in Case Management, Admissions, or Resource roles within hospice.
